Goal: Support client in processing grief, adjusting to loss, and maintaining connection with meaning, identity, and support.
Objectives:
Client will identify and express grief-related emotions, thoughts, and body experiences.
Client will explore the impact of loss on identity, relationships, routines, and sense of meaning.
Client will identify supportive rituals, coping strategies, and connection with memories or values.
Client will increase ability to tolerate waves of grief while continuing daily functioning as possible.
Interventions:
Therapist will provide grief-informed support and normalize varied grief responses.
Therapist will help client process emotions related to loss, including sadness, anger, guilt, numbness, longing, or relief.
Therapist will support meaning-making, continuing bonds, and adjustment to changed roles or routines.
Therapist will assess for traumatic grief, depression, or safety concerns as clinically indicated.
Goal: Increase client’s ability to navigate daily life, role changes, and practical adjustments following loss.

Client will identify ways the loss has affected daily routines, responsibilities, relationships, and sense of stability.
Client will develop manageable strategies for completing necessary tasks while honoring emotional capacity.
Client will identify support needs and practice communicating those needs to trusted others.
Client will explore changes in roles, identity, and expectations following the loss.
Interventions:
Therapist will support client in identifying practical and emotional stressors related to the loss.
Therapist will use problem-solving and pacing strategies to help client manage daily responsibilities.
Therapist will support communication skills for asking for help, setting boundaries, and naming needs.
Therapist will help client process role transitions and identity changes connected to grief.
Goal: Reduce isolation and support connection, meaning, and continued bonds after loss.
Objectives:
Client will identify patterns of withdrawal, isolation, or disconnection related to grief.
Client will explore meaningful ways to maintain connection with the person, relationship, role, or life chapter that was lost.
Client will identify supportive people, communities, rituals, or practices that reduce isolation.
Client will increase engagement in relationships or activities that feel supportive, meaningful, or grounding.
Interventions:
Therapist will normalize the non-linear nature of grief and the need for both connection and solitude.
Therapist will use grief-informed interventions to support continuing bonds, remembrance, ritual, and meaning-making.
Therapist will help client identify barriers to support, including guilt, fear of burdening others, or feeling misunderstood.
Therapist will support gradual reconnection with people, routines, or activities that align with client’s values and readiness.
